2) complete a registration form (click on the form to download a printable version):
 for normal registration
 for temporary registration (usually 2 months or less)
 for registering newborn babies
3) print and complete the Bingley Medical Pracitice Registration From
4) bring the completed forms to us.
When you come into the surgery, we will ask you to check your height, weight and blood pressure in one of our “Self Care” rooms.
5) if you are already taking regular medication, an appointment will be made for you to review this with one of our doctors.
