1) check that you live within our practice area. Click on the map below - if you live within the blue line, you can register with us.
2) complete a registration form (you can collect these from our Reception, or download them here...)

temporary registration
if you are away from home and staying in our area, you can register with us for up to 3 months for any health care needs that will not wait until your return home.

3) complete one of our own "new registration" forms (available from Reception, or download one here...)

4) bring your completed forms to our Reception.
When you come into the surgery, we will ask you to measure your height, weight and blood pressure in one of our automatic “Self Care” rooms.
5) if you are already taking regular medication, an appointment will be made for you to review this with one of our doctors.
